CSCS Enrollment Form

Every family that wishes to join the CSCS Umbrella Group must provide us with the necessary information and abide by our requirements by filling out our enrollment form, attached below.

Parent/Guardian Assent (initial):

I agree to meet all the requirements of Camp Springs Christian School and the State of Maryland in order to provide "regular, thorough instruction of sufficient duration" for my children.

If I do not meet these standards, I agree that I will be terminated from the CSCS Home School Program.
